Major Depressive Disorder (MDD)

Symptoms of Major Depressive Disorder (MDD)

The criteria for diagnosing Major Depressive Disorder as specified in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) includes at least five of the following symptoms being present for at least two weeks, with at least one of them being either (1) depressed mood or (2) loss of interest or pleasure:

  1. Depressed mood most of the day, nearly every day

  2. Significantly reduced interest or pleasure in all or almost all activities most of the day, nearly every day

  3. Significant weight loss when not dieting, weight gain, or decrease or increase in appetite nearly every day

  4. Insomnia or hypersomnia nearly every day

  5. Psychomotor agitation or retardation nearly every day

  6. Fatigue or loss of energy nearly every day

  7. Feelings of worthlessness or excessive guilt nearly every day

  8. Diminished ability to think or concentrate, or indecisiveness, nearly every day

  9. Recurrent thoughts of death, recurrent suicidal ideation without a specific plan, or a suicide attempt or a specific plan for committing suicide

  10. The symptoms cause clinically significant distress or impairment in social, occupational, or other important areas of functioning.

Diagnosis and Tests Diagnosis of MDD is typically based on a detailed personal interview, often using questionnaires and checklists to assist. There is no specific lab test to diagnose MDD. However, doctors may use lab tests to rule out other conditions that may have similar symptoms, such as thyroid disorders or vitamin deficiencies.

Treatment

Treatment for MDD can include a combination of medication, psychotherapy, and lifestyle changes. Selective serotonin reuptake inhibitors (SSRIs), serotonin and norepinephrine reuptake inhibitors (SNRIs), atypical antidepressants, tricyclic antidepressants (TCAs), and monoamine oxidase inhibitors (MAOIs) are types of medications that may be prescribed. Psychotherapies can include cognitive behavioral therapy (CBT), interpersonal therapy (IPT), and problem-solving therapy. Lifestyle changes such as regular exercise, a healthy diet, adequate sleep, and reducing alcohol and avoiding drugs can be beneficial.

Specialists Psychiatrists and psychologists are typically the professionals who diagnose and treat MDD. In some cases, a primary care doctor may also be involved in management and treatment.

Living With MDD Living with MDD can be challenging, but there are strategies that can help individuals cope:

  1. Therapy and Support: Regular appointments with a mental health professional and joining a support group can be beneficial. This can provide an opportunity to discuss experiences, feelings, and strategies for dealing with MDD.

  2. Healthy Lifestyle: Regular physical activity, balanced nutrition, adequate sleep, and avoidance of alcohol and drugs can help manage symptoms.

  3. Mindfulness and Relaxation Techniques: Activities such as meditation, yoga, and deep breathing can help to manage stress and improve mood.

  4. Self-Care: Taking time for enjoyable activities and hobbies, practicing good hygiene, and maintaining a regular daily routine can be helpful.
